Scott Rifkin, MD CEO, Mid-Atlantic Health Care, LLC Dr. Scott Rifkin is the founder and CEO of Mid-Atlantic Health Care, LLC, a leading provider of skilled nursing care and post-acute services. Dr. Rifkin has more than thirty years of experience in the practice of internal medicine and clinical healthcare management. As CEO, Dr. Rifkin oversees the operations of 18 Mid-Atlantic skilled nursing facilities with a combined total of 3,200 beds in Maryland, Delaware and Pennsylvania. Mid-Atlantic has a significant presence in Pennsylvania, where the company is the largest provider of skilled nursing care in Philadelphia and the owner of Parkhouse care center in Montgomery County. The company operates 1,700 beds in the Philadelphia market and 2,400 beds total in Pennsylvania.

Dr. Rifkin established Mid-Atlantic Health Care in 2003 and has since founded Mid-Atlantic affiliated companies which provide an array of healthcare solutions including data mining technology, network development and physician services. Real-Time Medical Solutions (RTMS) has garnered national innovation awards for its data mining software that detects subtle healthcare problems in electronic medical records and prompts appropriate clinical interventions.

National Post-Acute Healthcare manages bundled payment programs and is an approved convener of the Center for Medicare and Medicaid Innovation (CMMI)’s Model 3 Bundled Payments for Care Improvement Program (BPCI). That company also creates and manages postacute SNF networks for hospital systems. Five Star Physicians, another Mid-Atlantic Company, employs 60 full-time physicians who provide services to skilled nursing facilities. The combined companies are market leaders in their ability to help health systems manage the SNFs in their catchment areas.

Under Dr. Rifkin’s leadership, Mid-Atlantic has developed partnerships with hospitals and skilled care providers to improve patient health outcomes and overall population health. This includes a focus on innovative strategies to reduce unnecessary re-hospitalizations and return patients to a home setting when possible. Dr. Rifkin earned his medical degree from the George Washington University School of Medicine. He is certified in Internal Medicine and practiced medicine in Maryland for 20 years.
